Required Demographics (include each item in it’s own field/row):
- Unique Email Address
- Prefix/Title (ex. Dr., Mr., Mrs., etc.)-where available
- First Name
- Last Name
- Suffix (ex. MD-if available)
- Mailing address (if you are offering postal mailings)
Optional Demographics:
- Mailing Address: Please provide as much info as possible (State, Postal/ ZIP and country at a minimum) for companies that need to include/exclude regions.
- Job Titles, Disciplines, etc.
File Requirements:
- Header of each column should match this file.
- Please avoid names in ALL CAPS on the prefix and name fields. All records should be in Proper Case for personalization (mainly the name field unless you are also offering a mailing list.
- Important: Please remove all opt-outs before uploading or identify them on the file.
