Required Demographics (include each item in it’s own field/row):

• Unique Email Address
• Prefix/Title (ex. Dr., Mr., Mrs., etc.)-where available
• First Name
• Last Name
• Suffix (ex. MD-if available)
• Country
• Mailing address (if you are offering postal mailings)

Optional Demographics:

• Mailing Address: Please provide as much info as possible (State, Postal/ ZIP and country at a minimum) for companies that need to include/exclude regions.
• Job Titles, Disciplines, etc.

File Requirements:

• The header of each column should match this file.
• Please avoid names in ALL CAPS on the prefix and name fields.  All records should be in Proper Case for personalization (mainly the name field unless you are also offering a mailing list.
• Important: Please remove all opt-outs before uploading or identify them on the file.
